Two Medication Abortion Regimens: Misoprostol-Only vs. Mifepristone + Misoprostol - An Honest Comparison

Both work. Mifepristone plus misoprostol is about 95–98% effective in the first trimester with fewer side effects; misoprostol-only is roughly 85–95% effective with more cramping and a higher chance of needing follow-up. We provide the combined regimen through 14 weeks and 6 days — in person in Virginia Beach, and by telehealth for women in Virginia.

Key Facts

Two medication abortion protocols exist: the combined regimen (mifepristone followed by misoprostol) and misoprostol alone. Both are recognized by major medical organizations, including the World Health Organization. They are not identical, and the differences matter — especially when you are choosing between telehealth services, some of which offer only misoprostol. This page gives you an honest comparison so that you make an informed choice, whichever option you choose.

Which is more effective?

Mifepristone + misoprostol Misoprostol only
Effectiveness (first trimester) ~95–98% through 10 weeks ~85–95%, varying by gestational age and protocol
How it works Mifepristone blocks progesterone; misoprostol empties the uterus Misoprostol alone, in repeated doses
Cramping and side effects Significant, but generally less intense Typically, more cramping, nausea, chills, diarrhea; repeated dosing
Chance of needing follow-up Lower Higher (more incomplete abortions)
Regulatory status FDA-approved protocol; mifepristone is dispensed under a federal program Off-label but medically recognized (WHO)
How you can get it from us In person through 14 weeks and 6 days in one visit; by telehealth for women in Virginia through 14 weeks, and 6 days, after a short video visit, with pharmacy pickup or mail to a Virginia address Our fallback regimen (through 12 weeks) if federal rules end mail and pharmacy dispensing of mifepristone

What do the differences mean in practice?

Effectiveness

The combined regimen completes the abortion without further treatment roughly 95–98 times out of 100 in the first trimester. Misoprostol alone is effective roughly 85–95 times out of 100, depending on gestational age and dosing. Put differently: of 100 women, several more who use misoprostol alone will need a repeat dose or a follow-up procedure. Most won’t, but the odds differ, and you should know that before you choose.

The Experience

Misoprostol-only regimens typically involve repeated doses over several hours and tend to produce stronger cramping and more side effects. The combined regimen is usually a somewhat easier physical experience. Neither is easy; both are safe for the overwhelming majority of women.

Follow-up

With any regimen, have a plan for confirming the abortion is complete and for care if it isn’t. That is straightforward when a clinician is involved from the start — and it is why our telehealth service comes with our Virginia Beach clinic behind it.

Why do some telehealth services offer only misoprostol?

Mifepristone is governed by a federal program that controls who may dispense it and how. Those rules have been in litigation and under regulatory review, and mail availability of mifepristone has been — and may continue to be — subject to change. Misoprostol is an ordinary prescription medication with many uses; it is not subject to those mifepristone-specific restrictions and can be prescribed to a pharmacy or mailed without a special program.
We made a different choice. Our telehealth service for women in Virginia provides the complete mifepristone regimen through 14 weeks and 6 days today, because it is the better regimen and it is legal to provide. If federal rules end mail and pharmacy dispensing of mifepristone, we will switch telehealth to a misoprostol-only regimen through 12 weeks, and women near Virginia Beach will be able to keep the complete regimen by picking it up at our office. We will tell you plainly on our status page if that happens.
